I understand that I am making a commitment to the length of the 15 week semester, and that missed classes may be made up only by visiting other fall classes if one exists and provided the class size is not exceeded.I understand that there are no refunds, except if there is insufficient enrollment for the class to make.
I release any and all rights and claims for damages against Kindermusik of The Woodlands and its staff in the unlikely event of injury sustained by myself or my child(ren) during the course of or as a result of this musical activity.
I agree to pay the late fee of $15 if my balance is not paid by the DUE DATE of Sept. 2 or if I fail to set up a payment plan by SEPT 2. (Late Registrants are not charged a late fee.) I agree to pay the late fee of $15 if my credit card is declined on a payment plan. Please be sure to give us a credit card that is good through Nov. 7.
I agree to pay a $25 NSF Fee if my check is returned for insufficient funds and I agree to bring a replacement check or credit card within 7 business days.

I agree to give written notice should I need to withdraw from the semester due to unusual circumstances. I understand that the Down Payment is forfeited. I agree to have a $40 withdrawal fee applied against the remaining tuition. The remainder of the tuition after withdrawal fee, down payment and tuition at the point in time where notice was given will be credited to my in-house account for up to 12 months.
